It calls for a shift towards community-based care, better post-diagnostic support, and greater investment in prevention—arguing that these changes could help people live well for longer while reducing pressure on hospitals and carers.

The report also highlights the promising PriDem model, which integrates primary care, personal care planning, and community support. In pilot areas, it’s already helped more people receive a tailored care plan.
