This report highlights the urgent need for improved support for the nearly one million people living with dementia in the UK. Backed by the Alzheimer’s Society and the Peter Sowerby Foundation, the report reveals that current care is often patchy, unequal and under-resourced.
It calls for a shift towards community-based care, better post-diagnostic support, and greater investment in prevention—arguing that these changes could help people live well for longer while reducing pressure on hospitals and carers.
The report also highlights the promising PriDem model, which integrates primary care, personal care planning, and community support. In pilot areas, it’s already helped more people receive a tailored care plan.
